In a safety-critical industry like rail, the way incidents are investigated defines the
strength of its safety culture. A fair-culture approach shifts the focus from blame to
learning, ensuring that all parties, whether directly employed or contracted, are treated
equally and that investigations aim to uncover why something happened, not just who
was involved.

The Fair Culture Flowchart, introduced by Network Rail’s Safety and Sustainable Development division, provides a structured way to assess behaviour following an incident. It considers the individual’s intent, foresight, decision-making, and history, through four sequential tests.
